I/We hereby agree to the following:

1.  That I am participating in the training, programs, events, and dance classes offered by Gabrielle Almgren or alternate instructor, during which I will receive instruction.

2.  I understand it is my responsibility to consult with a physician prior to & regarding participating in any dance class. I represent and warrant I am physically fit and have no medical condition that would prevent my full participation in this class/lesson/event.

3.   In consideration of being permitted to participate in any dance class/lesson/event, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might incur as a result of participating.

 

4.   In consideration of being permitted to participate in any dance class/lesson/event, I knowingly, voluntarily, and expressly waive any claim I may have against Gabrielle Almgren, Gabrielle's Step by Step Dance Studio, or any dance instructor for injury or damages that I may sustain as a result of participating.

5.  I, my heirs or legal representatives forever release, waive, discharge, and covenant not to sue Gabrielle Almgren, Gabrielle's Step by Step Dance Studio, or any dance instructors for any injury or death caused by their negligence or other acts.
